Information Collection Specific to the Living PAH program
What Information is Collected?
We collect contact information, including your first and last name, e-mail, phone number(s), and mailing address. We also collect your gender and what UTC products (if any) you are currently receiving to treat your PAH (optional information). From time to time, we will ask you to respond to surveys, which helps us evaluate the program.
How the Information is Used
We respect the information you submit. Your information will be used only for operation of the program and internal program monitoring. This may include looking at program participation trends and discontinuation. Program staff will also use survey data to monitor trends in website user attitudes toward and satisfaction with the program.
We will never tie your name with your other personal information in our reporting and evaluation of the program. We will not use, sell, or assign your personal information for any purpose other than to communicate with you about the Living PAH program, and to administer the Living PAH program.
DO WE EVER SHARE PERSONAL INFORMATION WITH THIRD PARTIES?
